What is my Summary of Care Record and why does Leva Clinic need it?

Your Summary of Care Record (SCR) contains information about you and the care you have received. At Leva Clinic, we require medical care records for all new patients so your consultant is able to review these documents before your initial consultation. Should your consultant require any further documents, they will highlight this to you in your appointment.

Can I post my Summary of Care Record to Leva Clinic?

As we are a fully online clinic, we unfortunately cannot accept paper copies of medical records. We therefore ask that when you contact your GP, you ask for an online copy of your medical records which you can then forward to us via email to [e-mail].

How can I request my Summary of Care Record from my GP?

You can telephone or email your GP practice and request that the medical secretary sends this information via email to [e-mail].

You may use the following template to request your record:

"Dear [insert GP practice here],

I would like to request a copy of my Summary of Care record.

Please email this to me as soon as possible so I can manage my care. 

Best wishes,"

How long can it take for my GP to send me my Summary of Care Record?

Your GP must respond to your request within 28 days, but most requests are completed before this deadline. If you are experiencing delays in receiving your Summary of Care, it would be best to follow this up directly with your GP.

What if my GP will not send me my Summary of Care Record?

Sometimes, GP surgeries prefer the request for your medical records to come from the clinic which will be providing you with care. Once you have signed our 'consent for the release of medical records' form, we contact your GP to notify them you will receiving care under Leva Clinic and to request a copy of your Summary of Care Record.
